Course Description

Documentation Boot Camp: Forging Documentation Warriors is a 3-hour continuing education course for physical therapists, occupational therapists, and speech-language pathologists focused on strengthening the clinical, regulatory, and ethical foundations of documentation practice. Participants will learn to establish medical necessity, write SMART goals grounded in evidence-based practice, apply current Medicare and payer documentation requirements, and approach the SOAP note as an ethical act that captures both objective clinical findings and the patient’s own voice.

Course Objectives

At the end of this course, the participant will be able to:

• Define evidence-based documentation and apply its three pillars (research evidence, clinical expertise, and patient values/experience) to clinical notes
• Differentiate functional decline from underlying impairment across PT, OT, and speech-language pathology documentation
• Construct SMART treatment goals using the MICE framework
• Apply the Blueprint for a Skilled Statement to justify the need for a skilled provider across all three disciplines
• Identify how core payer and coding rules (including modifier use, therapy thresholds, and plan of care certification) intersect with documentation integrity
• Connect key medical necessity and documentation concepts to their corresponding ethical principles (beneficence, nonmaleficence, autonomy, justice, veracity, and fidelity)
• Identify the ethical obligations shared by therapists, employers, and payers within the documentation and reimbursement process
• Construct an evidence-based, ethically grounded appeal letter in response to a claim denial
