Exercise Prescription as Medicine for the Geriatric Patient: Make it Meaningful to Improve Adherence and Outcomes
Course Description
This course bridges the gap between clinical assessment and patient-centered outcomes by integrating the 5M Framework (Mind, Medication, Mobility, Multi-complexities, and What Matters Most). Participants will master the Patient-Specific Functional Scale (PSFS) to boost adherence, alongside appropriate dosing and intensity assessments to move beyond “3 sets of 10” for real physiological adaptation. Learn to use objective measures to collect data which will establish a baseline, validate progress, drive patient buy-in, and ensure treatment is relevant to daily life.
Course Objectives
At the end of this course, the participant will be able to:
- Recognize the benefits of mobilization in patient outcomes and decreased rehospitalization risk
- Apply the 5 M framework when prescribing an exercise program
- Integrate the Patient Specific Functional Scale into the assessment to improve patient buy in and follow through
- Prescribe a home exercise program based on What Matters Most to the patient based on specific assessments and with the appropriate intensity to accelerate improvement
