Multifactorial Strategies of Home Modifications
Course Description
In today’s healthcare climate, it is critical for clinicians to identify early, the additional barriers that will prevent the ease of transition to the home in order to prevent falls and possible readmissions to the hospital. In this 3rd of a 3 part home modification webinar series this introductory level course will discuss home modifications barriers. Caregiver strategies for home modifications and ability to perform ADLs will be discussed for those with dementia. This component will identify the role of sensory loss, communication and cognition barriers related to home safety and possible resources available for modifications. Funding resources will also be explored during the presentation. Case examples and internet video resources will be conducted to demonstrate application of the principles.
Course Objectives
At the end of this course, the participant will be able to:
1) Explain and identify physical barriers and possible recommendations to remove them related to safe discharge planning and transition home and community
2) Identify various types of resources for patients to investigate for home modifications, including design programs and funding resources.
3) Identify sensory barriers and possible recommendations related to safe discharge planning and transition to care in the home and community.
