Staying Home, Using Medical Screening to Prevent Hospitalizations
Course Description
This course explores the dimensions for rehabilitation professional’s role in keeping patients under their care from being rehospitalized. In the context of the Geriatric 5Ms Model, presenter will explore medical screening using a head-to-toe workflow covering prevalent co-morbidities, ensuring vitals are vital, including trending the trend and absolute versus relative stability. This course will tie frequently prescribed medications with co-morbidities placing a person at risk for hospitalization. Participants will reflect on how the information can be applied to their treatment settings.
Course Objectives
At the end of this course, the participant will be able to:
- Identify major medical health conditions, and associated pharmacology, increasing a patient’s risk of rehospitalization.
- Apply the Geriatric 5Ms Model to patient care coordination in rehab treatment settings to reduce rehospitalization risk for older adults.
- Describe best practices across rehabilitation practice settings to maximize absolute and relative stability and trend vitals of patients at risk for rehospitalization.
- Recognize interdisciplinary communication strategies that facilitate higher-quality patient care
