The Home Health Therapist: How to Improve Your Admission Workflow Room by Room
Course Description
Completing a home care admission is a complex and cumbersome task. A consistent workflow from chart review through documentation will enhance efficiency. Having a precise room by room template allows the clinician to solidify accuracy of the Start of Care. Using homogeneous workflow with each patient provides the clinician with room for the unexpected and prevents omitting needed SOC details. In the current healthcare environment where Value Based Purchasing is front and center, clinicians are performing a balance act between delivering high quality patient care and efficiency. The clinician also needs to layer in an excellent patient experience and show significant patient improvement. This course will present a room-by-room workflow with OASIS assessment items as well as other important items in the conditions of participation to equip a clinician to perform the start of care including all the details needed.
Course Objectives
At the end of this course, the participant will be able to:
- Explain the Home Health Admission process
- Demonstrate an effective, efficient and comprehensive assessment including body system assessment and OASIS items using room-by-room workflow
- Explain how to integrate workflow techniques to improve quality, patient experience and decrease ACH/ED visits.
- Critique individual assessments skills, identify inefficiencies and implement techniques to improve skills
